MND and Me Team

The MND and Me Foundation has always worked towards ensuring all Queenslanders with Motor Neurone Disease receive equitable access to support services when and where you need them. We try to offer you and your family support services that deliver the right solutions, at the right time.

A key support service is providing “Support Coordinators” to help guide and assist you access the services needed to maintain quality of life. This service operates under the name of “Spark NeuroCare”, it is run in partnership with MS Queensland

Support Coordinators works out in your community. They are on the ground and in the homes of people living with MND – assisting with:

  • Advice, information and resources
  • Links to local service providers and suppliers of aids and equipment
  • Referral to wellness and educational programs and events
  • Referral to allied health services such as physiotherapy, occupational therapy, speech therapy and dieticians
  • Ways to manage everyday life
  • Accessing National Disability Insurance Scheme (NDIS) or My Aged Care
  • NDIS support coordination.

We currently have Support Coordinators helping people with MND in the below regions:

  • Brisbane
  • Sunshine Coast
  • Bundaberg
  • Rockhampton
  • Cairns
  • Ipswich
  • Toowoomba
  • Gold Coast
  • Townsville

This means that approximately 84.8% of the Queensland population have geographical access to a Support Coordinator. The Foundation's goal is to expand this service to a truly state-wide program in the not too distant future to ensure all Queenslanders have access to best practice care and support.

Queensland is a large state that creates significant geographical challenges when planning a state-wide coordinated care service that has the capacity to reach as many people living with MND as possible.

The Foundation has partnered with MS Queensland to develop this Support Coordination service. This approach means the Foundation will be able to share costs, take advantage of existing infrastructure, as well as share knowledge and expertise to deliver the highest quality service to people living with MND.
